Molina Healthcare of Texas is hiring several Community Connectors. These roles are currently remote; however, candidates must reside in Texas. These positions will handle the initial outreach to our new members who are pregnant or postpartum to complete certain health risk assessments, assisting with resources (transportation, food, VAS benefits, connecting with providers, etc.). Community Connectors will also be trained to assist other members as well. Overall, these roles support community-based member advocacy activities. Serves as a local member advocate and resource, using knowledge of the community and resources available to engage and assist vulnerable members in managing health care needs. Contributes to overarching strategy to provide quality and cost-effective member care.
